My services include evaluation and medication management for adults age 21 and older.   After the initial psychiatric evaluation, we will talk about recommendations and formulate a treatment plan.

*  I do not prescribe Suboxone (buprenorphine/naloxone) at my practice.  I also do not do disability assessments.

Payment Information:
Please be aware that I do not accept any kind of insurance.  This means that I am an out-of-network provider for any kind of insurance reimbursement.  I can provide you with a receipt of service and the information you need to self-file a claim with your insurance company.  As the insurance company will reimburse you directly, full payment is expected by cash, check or charge at the time of service.

Patients with Medicare insurance as either a primary or secondary insurance may see me, but they would not be allowed by Medicare to self-file a claim for reimbursement.  Patients with Medicaid insurance are not allowed to see a provider who is out-of-network under any circumstance.
